Forms 2009-2010

At commitment night you will also need to turn in the following documents:

• Club Dues
• Uniform Sizes
• Code of Conduct
• Medical Release Form - has to be notarized. We will have one notary at the meeting.
• Prior to the meeting please register with Gateway using the following instructions below:

WEBPOINT

The webpoint membership application and fees must be submitted prior to participation in any practices or sanctioned event.

After tryouts and commitment night - Change the club name from "undecided" to 4 Rivers.
